  • 1. Fill out a claim

    Click this link to complete the claim form online, which will prompt you to attach your receipt before submitting.


    If you would prefer to print and mail in your claim form, you can download the form by clicking this link.

  • 2. Attach your receipt

    We’ll include a receipt with your shipping confirmation email. Chat us or visit a store if you need a new copy. 

  • 3. Submit claim and receipt

    If you are mailing in your claim form, be sure to mail your form and receipt to the following address:

    Note: Receipts for single-vision glasses with polycarbonate lenses do not break down the cost of frames and lenses separately because they are a packaged item. If your insurance carrier has questions, please do not hesitate to reach out to us!

  • 4. Get money back

    After you’ve placed your order and submitted your claim, you can get reimbursed. (This typically takes 2–3 weeks, depending on your carrier.) It’s that easy!
